How we picked: we compared only plans whose [policy wordings or prospectuses we have read](/about/methodology), on the five things that hit seniors hardest: entry age, co-pay, room rent, pre-existing disease (PED) wait and restoration. We then looked at each insurer's claim record. Insurer claim settlement ratios are FY 2024-25 health claims settled by number, from the IRDAI Handbook 2024-25 (Table 53) or the insurer's Form NL-37; see our [claim settlement ratio table](/articles/health-insurance/top-claim-settlement-ratio-companies).

## The ranking at a glance

| Rank | Plan | Entry age | Co-pay | Room rent | PED wait | Insurer CSR FY 2024-25 |
|---|---|---|---|---|---|---|
| 1 | [Care Supreme](/health-insurance/care-health/care-supreme) | 18+, no upper limit | None mandatory | No cap (base plan) | 36 months (can be cut to 1-2 years) | 96.85% |
| 2 | [Star Health Assure](/health-insurance/star-health/assure) | Up to 75 | 10% if you join at 61+ | Any room except suite at Rs 10L-25L; 1% of SI a day at Rs 5L-7.5L | 36 months | 86.42% |
| 3 | [Niva Bupa Senior First](/health-insurance/niva-bupa/senior-first) | Not yet verified | 50% default, can be cut to 20% | Not yet verified | 24 months | 92.55% |
| 4 | [Star Senior Citizens Red Carpet](/health-insurance/star-health/senior-citizens-red-carpet) | 60 to 75 | 30% on all claims | Capped (1% of SI a day up to Rs 5L) | 12 months | 86.42% |

Star's 86.42% is below our 90% flag line, and Niva Bupa's 92.55% sits in our "acceptable" 90-95% band. Care is above 95%, our "strong" band. See the [methodology](/about/methodology) for the bands.

## 1. Care Supreme: best overall for seniors

Care Supreme accepts adults at 18 and above with no upper entry age. Its base plan has no room-rent cap and no mandatory co-pay, offers Unlimited Automatic Recharge, and pays a 50% a year cumulative bonus that a claim does not reduce. The PED wait is 36 months, but an add-on can cut it to one or two years. Avoid the Smart Select discount for a parent unless their hospitals are on its list, because it adds a 20% co-pay elsewhere. Acceptance at older ages depends on medical underwriting.

## 2. Star Health Assure: entry up to 75, unlimited restoration

Star Health Assure accepts entry up to 75 and restores cover an unlimited number of times a year, with 180 days of post-hospitalisation cover. Anyone joining at 61 or older pays 10% of every claim. Buy at least Rs 10 lakh: below that, room rent is capped at 1% of sum insured a day.

## 3. Niva Bupa Senior First: shortest wait among the full plans

Senior First's PED wait is 24 months, and its ReAssure benefit reinstates cover without limit after the first claim. The catch is the default 50% co-pay. You can lower it to 40%, 30% or 20% when you first buy, for a higher premium, or choose an annual deductible instead. A room above your eligible category adds another 10% co-pay. We have not yet verified its entry age or room eligibility from the wording.

## 4. Star Senior Citizens Red Carpet: the fallback

Red Carpet is built for 60-75 year-olds with lifelong renewal and a short 12-month PED wait. In return you pay 30% of every claim, room rent is capped (Rs 6,000 a day at Rs 7.5 lakh to Rs 10 lakh cover), post-hospitalisation is limited to 7% of the bill, and sub-limits apply to conditions such as cataract and cancer. Use it when a fuller plan will not accept your parent.

## Also consider

[Star Comprehensive](/health-insurance/star-health/comprehensive) accepts entry up to 65, covers a single private room, and has a buy-back option that cuts the PED wait to 12 months. Joining at 61 or older adds a 10% co-pay.

## How to choose

1. List every existing condition and medicine. Then compare [waiting periods](/articles/health-insurance/health-insurance-waiting-periods) and the PED rules.
2. Work out what a 30% or 50% co-pay would mean on a Rs 5 lakh bill. See co-pay vs deductible.
3. Prefer no room-rent cap. Read room rent and proportionate deduction.
4. Read the wider senior citizen health insurance guide and our parents list.
